What you pay them
Annual amounts. If you don't know a number, that's an answer too — mark it "No idea" and leave the amount blank or estimated. Your Form 5500 Schedules A and C hold several of these; your CFO holds the rest. Then the harder blank under each one: name what that vendor actually delivered this year, in one sentence. If the sentence won't come, notice that.
What it saved you
Documented savings from all of the above, for the same year. Not projections, not "trend mitigation," not a repricing report — dollars you could show an auditor.
